The following notes should be read in conjunction with the information available on the Application form or at www.passports.gov.au.
1. You can complete an application form online at www.passports.gov.au or request one from the Australian Embassy. NOTE: Adult applicants who have a current Australian Passport or one which has expired in the last 12 months may be eligible to use the one page application form. Contact the Embassy or visit www.passports.gov.au for more information.
2. Please use a BLACK PEN and print in BLOCK LETTERS within the boxes when completing the application.
3. All certificates must be ORIGINALS. Italian certificates (birth, marriage, death etc) must be issued in the “international” form (formato internazionale or plurilingue) or be accompanied by an official English translation.
4. Proof of citizenship:
a) Full Birth Certificate . If born after 20/8/1986 you will also need to provide proof that one of your parents was an Australian citizen or permanent resident of Australia at the time of your birth.
or
b) Australian citizenship certificate. If your citizenship certificate does not detail date and place of birth, you must also provide your original birth certificate. If you cannot provide a full birth certificate, you should provide a foreign passport showing date and place of birth. Please contact us if neither of these can be provided..
5. A marriage certificate is required only for women who choose to apply in their married name for the first time or wish to revert to their maiden name.
6. "Declaration by Guarantor" (section 12) must be completed and signed by a person who is either an Australian passport holder OR is one of the following professionals: doctor, solicitor, priest, public servant, police officer, teacher or chartered accountant with a minimum of 5 years service and who has known the applicant for at least one year. Guarantors who hold an Australian passport do not need belong to the above group of professionals and should complete Section 12 A only of the application form.
7. Provide two photographs with the completed application form. One must be endorsed by the guarantor who has completed section 12 of the form. Please check page 4 of the form for photograph size and requirements. You must have a neutral expression, i.e. DO NOT smile or show teeth while having the photos taken.
8. Provide the Australian passport you are seeking to replace. (i.e., your most recently issued passport) unless it has been lost or stolen.
9. Provide TWO identification documents. One from each list below:
a. Driver's licence OR government issued identity card OR permesso di soggiorno
b. Codice Fiscale OR foreign passport OR government health card OR bank card.
In the case of minors, one of the above documents bearing signature and photo is required from each parent.
10. Applications: Applicants applying to replace an Australian Passport which was issued after 20/08/1986 with a validity of at least two years may apply by mail unless they fall into one of the following categories, in which case, attendance is compulsory:
• First applications. Attendance is compulsory if you are applying for your first Australian passport. In the case of minors one of the parents must attend to lodge the application (The minor need not attend);
• Applications from adults who are applying for their first passport since turning 18;
• Applications for the replacement of a passport that has been *lost, stolen or damaged and in the case of applications for the replacement of an Emergency Passport.
11. Your new passport can be either returned to you by post or collected in person. The original receipt and authorisation must be produced if the passport is collected by someone other than the applicant. An additional fee of €4.00 applies if the new passport is to be delivered to you by registered mail.
12. The correct fee must accompany the applications. The fee can be paid in cash (for applications in person only) or with an Italian bank cheque (assegno circolare) payable to the AUSTRALIAN EMBASSY.

Note: Please note that all fees are payable in Euro only and cannot be refunded.
*Additional fees apply if you have had a lost or stolen passport in the last five years.
