The following Information Notes should be read in conjunction with the information available on the PC7 Passport Renewal Application form or at www.passports.gov.au
1. Sections 1, 3, 4, 5 and 6 have to be completed. Should any part of the application be incorrect it will be returned unprocessed.
2. Please use a BLACK PEN and print in BLOCK LETTERS within the boxes when completing the application.
3. The photographs supplied must meet the requirements (see reverse side of the application form for samples). Please note that you must have a neutral expression, i.e. NOT smiling nor showing your teeth).
4. Your Australian Passport must be included with your Renewal Form; it will be cancelled and returned to you with your new passport.
5. Sign within the white box in section 6 only.
6. Please do not write in the grey section.
7. The form must be sent to our office together with 1) your passport, 2) two photographs, 3) a photocopy of an identity document showing photograph and signature, and 4) payment in the form of “assegno circolare” made payable to “Australian Embassy”.

Note: Fees are subject to change and cannot be refunded. Additional fees will apply if you have had a lost or stolen passport in the last five years.

Le seguenti note dovranno essere lette quando si compila il modulo di rinnovo Australian Passport Renewal Application - PC7. Altre istruzioni sono presenti sia sul modulo che sul sito www.passports.gov.au. .
1. Vi preghiamo di compilare le sezioni 1, 3, 4, 5 e 6. IMPORTANTE – Se la domanda non è completa verrà restituita all’interessato
2. Scrivere con penna NERA e a stampatello senza uscire dai riquadri bianchi.
3. Le FOTOGRAFIE devono essere conformi alle istruzioni: vedere il retro del modulo (Si prega di fare attenzione a NON sorridere o mostrare i denti quando si fanno le foto per il passaporto).
4. Il vostro passaporto australiano deve essere spedito assieme alla domanda di rinnovo; sarà quindi annullato e vi sarà restituito assieme al nuovo passaporto.
5. La firma si pone solamente nella sezione 6 e non deve uscire dal riquadro bianco.
6. Vi preghiamo di non scrivere nella sezione grigia..
7. Il modulo dovrà essere spedito con il passaporto australiano, due fotografie, una fotocopia di un documento d’identità recante foto e firma ed il pagamento in forma di assegno circolare intestato a “Australian Embassy
8. Si prega di chiamare prima di spedire il pagamento per controllare che il prezzo non sia variato. La quota pagata NON è rimborsabile. Una quota aggiuntiva viene applicata a chi ha perduto un passaporto valido negli ultimi 5 anni.
Per ricevere il passaporto a casa tramite Posta Raccomandata bisogna aggiungere €4.00
