EXPLANATORY NOTE REGARDING COMPLETION OF DIAC FORM 1229 FOR TRAVEL TO AUSTRALIA BY WORLD YOUTH DAY PARTICIPANTS
Form 1229 provides consent by parents or legal guardians to the grant of a visa to a child under the age of 18.
The parents or legal guardians of children applying for a Tourist (subclass 676) visa to participate in World Youth Day events in Australia must complete form 1229. The completed form must be provided to the office at which the Tourist visa application is being made, either electronically (see instructions on page 1 of the form) or by mail.
Note: Form 1229 is NOT required if you are applying for an Electronic Travel Authority (ETA) to travel to World Youth Day.
The Australian Government requires parents or legal guardians to give their written consent for the grant of a visa to a person under 18 years of age. An electronic or paper Tourist visa application for a person under 18 years of age must be refused if Form 1229 is not provided.
For all World Youth Day participants applying for an electronic (eVisa) Tourist visa, or sending their application by mail to the Australian Embassy in Rome, Parts A, B and C of Form 1229 must be completed (in English) by the parents or legal guardians of a visa applicant aged under 18 years of age. The signature required at Part C must be witnessed by an appropriate person. In Italy, for the purposes of providing this form for World Youth Day travel, an appropriate person includes:
doctor;
lawyer;
registered minister of religion (priest);
dentist;
nurse;
optometrist;
pharmacist;
physiotherapist;
full-time teacher;
bank manager or bank officer with 5 or more continuous years of service;
postal manager with 5 or more continuous years of service; or
police officer.
Parents or legal guardians should sign Form 1229 in front of a person holding a position in the list above, who should then sign Part C of the form as the ‘Witness for parent/legal guardian.
The signed form should then be sent to the office processing the child’s request for a visa to attend World Youth Day events, following instructions on page 1 of the form for electronic dispatch or by including it with together with an application.
Please note that applicants for an Electronic Travel Authority (ETA) do not need to complete form 1229.
